Protein C is a vitamin K-dependent plasma glycoprotein. It is cleaved to its activated form by the thrombin-thrombomodulin complex. Activated Protein C is a natural anticoagulant due to its action of proteolytically inactivating clotting factors Va and VIIIa.
Protein C deficiency is an inherited disorder characterized by an increased risk of VTE. There are qualitative and quantitative protein C deficiencies. Quantitative deficiencies account for 75-80% of all cases. Unfortunately, no single test for Protein C is 100% sensitive and specific for all the abnormalities.
In the chromogenic Protein C assay, patient’s platelet poor plasma is incubated at 37°C with the Protein C activator (Protac). After incubation, the chromogenic substrate for activated Protein C is added. The change is optical density at a wavelength of 405 nm is measured and by comparison against a standard reference curve, the Protein C level can be determined.
This assay is not influenced by the presence of a lupus anticoagulant, Factor VIII level or the presence of the Factor V Leiden mutation.
